- What
should
I
bring
for
my
appointment?
Referrals(if needed) are the responsibility of the patient. Please make sure proper referrals are obtained prior to your visit. Please be sure to bring the patients insurance card as well.
- What
if
I
am
unable
to
make
my
copay
at
the
time
of my appointment?
Copays and payment for any past due balances are due at the time of service. The appointment will be rescheduled if you are unable to pay the copay.

if
I
am
unable
to
make
my
scheduled
appointment
time?
You will get a reminder call 24-48 hours prior to your appointment. We request that if you know that you are unable to keep the appointment, please call our office.
- What
should
I
provide
if
I
am
a
new
patient?
If you are new to our practice, please click on the new patient and health history forms. These forms (in the right download area) can be printed and filled out at your convenience prior to your appointment. Please bring these forms with you on the day of your visit. If it is not possible to work on these forms prior to your visit, they can be filled out in the office. If you choose this option, we request that you arrive at least 15 minutes prior to your scheduled appointment time to allow sufficient time to complete all necessary paperwork. Please bring your referral (if needed) as well as your current insurance card and driver license.

ok
if
I
have
makeup
on
for
my
appointment?
If you are having any part of your face examined it would be better to remove any makeup prior to the appointment. If you are coming directly from work we have facilities for you to remove the makeup there.
