We would like to make the registration process for your office visit as efficient and uncomplicated as possible. The following tips can assist with the process.
New Patients Forms to Download

If you are new to our practice, please click on the new patient and health history forms. These forms (in the right download area) can be printed and filled out at your convenience prior to your appointment. Please bring these forms with you on the day of your visit. If it is not possible to work on these forms prior to your visit, they can be filled out in the office. If you choose this option, we request that you arrive at least 15 minutes prior to your scheduled appointment time to allow sufficient time to complete all necessary paperwork.

Please review and sign the HIPAA/Privacy Notice form in addition to the patient information forms. With new federal privacy regulations that have been enacted, these forms need to be completed for your appointment.

Established Patients

If you have not been seen in the office in 3 years, please fill out all of the above new patient forms and bring them with you to your appointment so that we can update your records.

If any pertinent health information has changed since your last visit, please fill out a new health history form.

Important Reminders for All Patients

Referrals (if necessary) are the responsibility of the patient. Please make sure proper referrals are obtained prior to your visit.

Bring a copy of your insurance card with you. Failure to present this card at the time of the visit will result in your being responsible for the full cost of services provided or you will be given the option of rescheduling the appointment.

Copays and payment for any past due balances are due at the time of service. The appointment will be rescheduled if you are unable to pay the copay.

You will get a reminder call 24-48 hours prior to your appointment. We request that if you know that you are unable to keep the appointment, please call our office.

A current medication list is helpful so that we can provide safe and accurate treatment.
